BCP Council seeks Support & Connect service for up to 12 weeks of 1:1 support.
Bournemouth Christchurch and Poole Council is commissioning a Support & Connect service to prevent, reduce, or delay long-term conditions and enhance outcomes for people. The service provides 1:1 telephone, face-to-face, or group support for up to 12 weeks, connecting individuals to local groups, activities, and support services using a strengths-based, person-centred approach. The contract is for 3 years with an option to extend for 2 years (3+1+1), totaling up to 5 years, starting February 2027. Suppliers must demonstrate experience in delivering similar community-based support services.
