The service targets older people at risk of falls who cannot access community-based programmes, using the Modified Otago Exercise Programme (MOEP). Key deliverables include personalized assessment, strength and balance interventions, home hazard assessment, education, referrals, and outcome data collection. Providers may be individual or collaborative, must cover the entire South Island including rural areas, and demonstrate culturally responsive delivery.
