Health New Zealand (Te Whatu Ora) is procuring a Regional In-Home Falls Prevention Service across Te Waipounamu (South Island). The service will deliver evidence-based strength and balance programmes based on the Modified Otago Exercise Programme (MOEP) to older people at risk of falls who cannot access community programmes. Key deliverables include personalised assessment, strength and balance interventions, home hazard assessment, education, referrals, and outcome data collection. The contract term is 5 years with a 2-year renewal option, covering rural and urban areas. Providers must demonstrate ability to deliver culturally responsive services and integrate with primary care and hospital services.
