NHS Bristol, North Somerset and South Gloucestershire Integrated Care Board (BNSSG ICB) is procuring a Community Rehabilitation Service for adults with complex and enduring mental health needs, primarily psychosis. The service aims to build independence, reduce relapse, minimize hospital admissions and out-of-area placements, and support stable community living. It includes two core functions: 24/7 residential rehabilitation at Wellbridge House (psychosocial interventions, medication management, physical health support, daily living skills) and community rehabilitation (structured rehab plans, case management, crisis planning, safeguarding, multi-agency coordination).
