Adult Social Care diagnostic and transformation plan for Herefordshire Council.
Herefordshire Council has awarded a contract for a time-bound, evidence-led diagnostic of its Adult Social Care service as Phase 1. The diagnostic will quantify financial and practice-based improvement opportunities, define a savings profile, and produce a fully costed transformation plan to improve outcomes for residents while reducing costs. The contract includes an option for Phase 2 covering design, implementation, and support for sustained improvements. The procurement was conducted via a further competition under the Crown Commercial Services RM6309 Management Consultancy Framework (Lot 3).
